1. Parties, testing & documentation

This agreement is between ENERGUYS INC.™ (Contractor / Service Provider) and the named homeowners for the property and claim shown above.

Benjamin McQueary will perform moisture testing to evaluate water intrusion and mitigation needs, using industry-standard moisture meters, thermal imaging and atmospheric / psychrometric measurement equipment appropriate to the conditions, operated according to manufacturer instructions and applicable IICRC practices. Record accessible-area moisture readings, affected materials, photographs and findings in a written report for the homeowner and carrier.

This authorization covers testing and documentation only—not demolition, drying/remediation work, construction, a statutory mold assessment, engineering or insurance adjusting.

2. Permission to bill the carrier directly

Homeowner authorizes ENERGUYS INC.™ to submit its itemized invoices, estimates, moisture readings, photographs and reports directly to the named insurance carrier and its authorized adjuster for this claim, and to communicate about its own services and billing.

Homeowner requests that the carrier pay the covered, undisputed amount for ENERGUYS INC.™ services directly to ENERGUYS INC.™ or name it as a co-payee, subject to the policy, carrier procedures and any mortgagee rights.

3. Insurance proceeds & agreed charges

Only proceeds attributable to ENERGUYS INC.™ authorized services are directed toward its properly due invoice—not the entire claim or other contractors’ work.

If those proceeds are paid to Homeowner, Homeowner agrees to remit the undisputed amount properly due to ENERGUYS INC.™ after funds clear, according to the separately accepted written payment schedule.

The authorized charge is $600–$850, depending on the testing and documentation needed, billed directly to the insurance carrier. The final itemized invoice must reflect the services actually performed and stay within this range.

Any charge above $850 or additional service requires Homeowner’s separate written approval before it is performed. No homeowner deposit is required by this agreement; any deductible or noncovered charge payable by Homeowner requires a separate express written agreement.

4. Rights retained & cancellation

This is not an assignment of insurance benefits, a power of attorney or authority to endorse checks, settle the claim, interpret coverage or sue the carrier in Homeowner’s name.

The carrier is not a party and is not required by this agreement to cover or pay any charge.

Homeowner may revoke future access, information-release and direct-billing authority by notifying ENERGUYS INC.™ and the carrier; no cancellation penalty is imposed by this authorization and statutory rights remain intact.

5. Coordination & acceptance

Any P1Mitigators™ fee to the homeowner is set only in a separate signed Homeowner Claims Support & Trade Support Agreement; any specialist-paid processing fee is separate from deductible and authorized service costs.

P1Mitigators™ is the mitigation software company providing trade-service support; it is not the testing provider or a contracting party.

Homeowner’s signature authorizes the limited scope and billing communications above and accepts the linked Full Terms; it does not represent that testing has occurred or that any invoice is approved.

ENERGUYS INC.™ must obtain separate written authorization before destructive testing or additional work and provide the homeowner with the resulting report and itemized billing.

7. Authorized $600–$850 service charge & billing

Homeowner authorizes $600–$850 for the moisture testing and documentation described in this agreement, depending on the work actually needed; ENERGUYS INC.™ bills that itemized charge directly to the insurance carrier.

The invoice must describe the services performed and the resulting charge within the authorized range; amounts over $850 and additional services require separate prior written approval.

Before on-site services begin, ENERGUYS INC.™ must confirm the appointment and provide its complete business mailing address and any legally required cancellation notice or form.

No appointment, service completion or contractor signature is supplied by P1Mitigators™.

Denial or delay of insurance payment does not itself create a new homeowner debt under this authorization; any homeowner responsibility must follow a separately accepted lawful service agreement.

All payments must be credited to the same invoice, with no duplicate collection or diversion of proceeds allocated to other work.

8. Limited records release & carrier authority

Homeowner permits ENERGUYS INC.™ to share its records reasonably necessary to substantiate this testing invoice with the carrier, its authorized claim representatives and P1Mitigators™ for administrative coordination.

Homeowner requests corresponding invoice status and service-related payment information be shared with ENERGUYS INC.™; this does not authorize release of unrelated personal records.

Homeowner retains policy rights, claim control and the ability to dispute charges or coverage decisions.

This agreement does not transfer ownership of policy benefits or grant ENERGUYS INC.™ independent standing to enforce the policy.

9. Revocation, copies & electronic signature

For scheduling or revocation, contact Benjamin McQueary at the ENERGUYS INC.™ phone shown in this agreement and notify the carrier of any change to billing instructions; keep a record of the notice.

Revocation applies prospectively and does not erase separately agreed lawful charges for services already performed or lawful record-retention duties.

Any applicable statutory cancellation rights control over inconsistent terms; signing does not waive those rights or authorize premature work.

Each signing homeowner consents to electronic records and signature and may print or save the short agreement and Full Terms.
