Why do retaining walls fail?

Almost always drainage and base. A wall built on poor base material with no gravel backfill or drainage pipe traps water pressure behind it, and that pressure pushes the wall out until it bulges, leans, and fails. Our contractors build the base and drainage correctly so that never starts.
